  • If you cannot find your insurance plan listed, you may bypass this field by selecting “cash”. Your insurance company will be billed as long as we have your current insurance information on record.

  • It may take up to two business days to receive confirmation that your appointment request was accepted.

  • On rare occasions, an appointment request may be declined due to a scheduling conflict. Should this happen, you may submit a second request online or you can message me through the patient portal to schedule.
